Newly Created UCs?

The most common reason is the UC was taken from a breached accounts and then traded for xyz
1. Original owner returns to neopets and finds their account was hacked and/or emptied, makes a ticket
2. TNT goes down the chain and reverses trades.
3. Some people (the only documented one I remember reading about had 2 people) in the chain are given chances to make replicas depending on how they were affected.

Yeah, usually it's the people who lose a UC in the chain that are given the opportunity to make replicas, although I've heard of people falling through the cracks in some reversals.
