I have the same question, just double checked and I have all of them cleared as well and is not working for me either.

I didn't have all the areas cleared of fog, so I went ahead and did my last one, and... still no achievement.
It must be seriously broken if that flag doesn't unlock it. :\

ya, it's stilled grayed out for me and I've cleared all of the fog :/
